Eclipse has been changed in 3 patches since release in Patch 5.2. 1 nerf, 2 adjustments. Most recently adjusted in Patch 6.2.

UNIQUE Passive - Ever Rising Moon: Hitting an enemy champion will inflict a stack for 1.8 seconds, up to one per attack or cast per champion. Inflicting 2 stacks will consume them all to deal bonus physical damage equal to 6%/3% target's maximum health and grant you a shield equal to 140/70 (+ 35%/18% bonus AD) lasting 2 seconds.

Unique Passive - Ever Rising Moon:

Hits include damaging basic attacks, abilities, item effects, and summoner spells, as well as inflicting crowd control and damage over time effects. Inflicting 2 stacks will consume them all to deal bonus physical damage equal to Melee 6% / Ranged 3% target's maximum health and grant you a shield equal to Melee 160 / Ranged 80 (+ Melee 40% / Ranged 20% bonus AD) lasting 2 seconds (6-second cooldown).
- Limited to 1 Eclipse.
